I am writing to provide you with a record of the outcome of your frozen embryos held in storage at [clinic].


One embryo was frozen on 5th November 2011. During this frozen embryo transfer cycle which commenced on 3rd January 2012, one embryo was thawed and transferred on 17th January 2012.

Please note that there are no embryos remaining in storage at [clinic].
