Letter from the lab

Dear A and D

Thank you for having IVF treatment at [clinic]. I'm writing on behalf of the laboratory team with a brief summary of the embryology from this cycle.

Date of egg collection - 22/09/14
Number of eggs collected - 14
Method of insemination - ICSI
Number of eggs suitable for insemination using ICSI - 10
Number of eggs fertilised normally - 5
Number of embryos transferred to the uterus - 1
Number of embryos suitable for freezing and frozen for future use - 1

We have followed your instructions on your consent form regarding any eggs and embryos not suitable for freezing.

The storage of frozen embryos is limited to ten years under the Human Assisted Reproduction Technology (HART) Act 2004. An application to extend storage can be made to the Ethics Committee on Assisted Reproductive Technology (ECART) at least 6 months before the 10 year period ends. Please remember to keep in contact with [clinic] and to tell us of every change of address so we can keep in touch while you have embryos, eggs or sperm stored.

You still have frozen sperm stored for the option of future treatment.
